Liposuction means the surgical aspiration of fat from the subcutaneous plane leaving a more desirable body contour. Liposuction is one of the most popular cosmetic procedures performed by Plastic Surgeons/ Cosmetic surgeons.
In the initial consultation, patient’s goal is assesed. The surgeon informs the patient, what can and what cannot be achieved with liposuction. Some patient may require alternative procedures like – Abdominoplasty, Liposuction combined with Abdominoplasty or Bariatric Surgery. Patient are explained that, liposuction is not a weight loss procedure. It is a body contouring procedure. Abnormally distributed bulges of fat or fat that is distributed outside the confines of the ideal body shape are the target that is commonly suctioned. Abdomen is carefully examined for hernia, significant abdominal wall laxity, abdominal scars, history of abdominal radiation, and anything that might affect abdominal wall integrity.
The surgery is carried out as an outpatient / daycare / Indoor basis depending on patient convenience /safety and amount of liposuction done. Depending on the type of instrument used, liposuction can be divided into 3 types –
1) SUCTION ASSISTED LIPOSUCTION (SAL) – It is considered the gold standard tool for liposuction. The technique uses varying diameter, blunt tip cannulas attached via large bore tubing to a source of high vaccume, which effectively suctions the fat through holes in tip of cannula. It requires a fair amount of physical effort on the part of the Cosmetic Surgeon.
2) ULTRASOUND ASSISTED LIPOSUCTION (UAL) - Here Ultrasonic energy produced in piezoelectric crystal within the handpiece is transmitted down the cannula to its tip, where the ultrasonic energy causes micromechanical, thermal and cavitational effect on subcutaneous fat. This requires less physical effort on the part of the Plastic Surgeon.
3) POWER ASSISTED LIPOSUCTION (PAL) – This is basically suction assisted liposuction, powered by a reciprocating cannula. The main advantages of PAL over SAL are its efficiency in fibrous areas and its ease of operation for the surgeon.
Liposuction is done in the following manner -- Tumescent fluid is infiltrated in the area selected. The wetting solution is allowed 10-15 minutes for maximal vasoconstrictive effects. The liposuction cannula is introduced through multiple small incisions, the location of which depends on the area being suctioned. Attempt is made to hide incision in anatomic creases or Langer cleavage lines, so scars are not visible. Cross tunneling is done for most areas to avoid contour deformity. After completion of liposuction; the end point of which is determined by – the contour of the patient, the aspirate volume & pinch test; the incision for cannulas larger than 3mm are generally closed with 5-0 nylon. The patient is advised to use a compression garment for couple of months
In Face and neck, Upper arm, Abdomen, Back, Breast, Posterior hips & flanks, Buttocks, Thighs, Calf, Knee area. Liposuction is also a good treatment in most cases of Gynaecomastia with/ or without gland excision.
Overcorrection, Under correction, contour abnormalities, Hematoma and seroma, can be minimized to a great extent by taking the right precautions during liposuction .
